The inquiry chairman Sir Adrian Fulford said that ‘this culture has to end’ after uncovering series of failures ahead of dance class attack after the first report into the attack condemned the services and people that cane into contact with Axel Rudakubna before his knife attack

Rudakubana was able to carry out the Southport atrocity because of “catastrophic” failures by multiple agencies and the “irresponsible and harmful” role of his parents the first report has found

Sir Adrian Fulford said the murder of the three young girls and stabbing of 10 others was not “a bolt of lightning out of a clear blue sky”. adding  “Instead, some form of grave violence … had been clearly, repeatedly and unambiguously signposted over many years.

“Indeed, several professionals who had direct contact with Rudakubana expressed serious fears, sometimes in stark terms, that he would go on to harm or kill.”

The report covers two volumes and sets out 67 recommendations

His parents come in for particular criticism

Alphonse Rudakubana and Laetitia Muzayire, had “created significant obstructions” to engagement with him by various agencies and failed to stand up to his behaviour and set boundaries, as well as failing to report a clear escalation in his risk.

Sir Adrian said: “If his parents had done what they morally ought to have done, AR would not have been at liberty to conduct the attack and it would not therefore have occurred.” But, he said it should have been “obvious” to those in a position to act that Rudakubana was not being “effectively parented”.

Commenting on the findings Kier Starmer said

“The brutal, senseless murders of Bebe, Elsie and Alice marked one of the darkest moments in our country’s history.

The report today is truly harrowing and profoundly disturbing. It sets out systematic failures that led to this terrible event.
